Reconstitution does not determine the prescribed dose. It changes how much medication is present in each milliliter of solution. Using the wrong amount of liquid can create a concentration different from the one your clinician intended, increasing the risk of a dosing error.

Not every peptide requires reconstitution. Some injectable products arrive as ready-to-use solutions, while troches, capsules, nasal sprays, and topical products use different preparation methods.

Before you reconstitute a peptide

Confirm all of the following before starting:

Do not inject products labeled “research use only” or “not for human consumption.” These products are not dispensed as medications and may not meet the sterility, identity, potency, or quality standards required for patient use.

What do you use to reconstitute peptides?

Use only the diluent identified by the medication manufacturer or dispensing pharmacy. Depending on the formulation, this may be Bacteriostatic Water for Injection, Sterile Water for Injection, sodium chloride solution, or a product-specific diluent supplied with the medication.

BAC water for peptides contains 0.9% benzyl alcohol and is packaged for multiple withdrawals. Sterile Water for Injection has no antimicrobial preservative and is generally supplied in a single-dose container. They are not automatically interchangeable, and neither should be replaced with tap, bottled, distilled, or homemade water.

How to reconstitute peptides step by step

The exact directions included with your medication always take priority. When a licensed pharmacy has instructed a patient to reconstitute a prescribed peptide, the general process is:

1. Read the pharmacy instructions completely

Verify the medication, strength, diluent, volume, storage conditions, and beyond-use date before opening any supplies. Do not proceed if a number is missing or unclear.

2. Prepare a clean workspace

Wash and dry your hands. Work on a clean, dry surface away from food, sinks, pets, and other possible sources of contamination. Gather every item before beginning.

3. Inspect both vials

Check the seals, labels, expiration dates, and appearance of the medication and diluent. Do not use a vial that is damaged, leaking, unexpectedly cloudy, discolored, or contains particles.

4. Clean the vial stoppers

8. Inspect and label the solution

The total medication in the vial remains unchanged, but every measured volume will contain a different amount.
